Create and Customize a Review Form

As an administrator, you can customize questions to tailor the form to the type of review being conducted and the role of each participant.

Written by Aurélia MICALLEF

1. Add questions to a form

From the administration area, go to the form library and open the form you want to configure.

You can organize the form into several sections and add different questions depending on the information you want to collect.

You can also duplicate an existing form.

Each section must contain at least one question for the form to be saved.

The following question types are available when creating a review form:

Question type

Description

Free text

Allows the employee or reviewer to enter a written response.

Single choice

Allows participants to select one answer from several options.

Multiple choice

Allows participants to select several answers from the available options.

Dropdown list

Allows participants to select a value from a predefined list.

Rating scale

Allows participants to assign a rating based on a configured scale.

Numeric

Allows participants to enter a numerical value.

Date

Allows participants to select or enter a date.

Skills

Allows employees' skills to be assessed, whether related to their position or their overall profile.

Objectives

Allows objectives to be assessed as part of the review.

Training

Will allow participants to select training courses related to their development needs. This feature is currently being finalized.

Additional features

In addition to selecting the question type, administrators can configure:

  • Conditional questions: Display a follow-up question based on a previous answer.

  • Required or optional questions.

  • Role-based targeting: Employee, manager, or both.

  • Customized question labels based on the participant's role.

  • Validation rules: Text format, email address, URL, or numerical constraints.

2. Create conditional questions

Conditional questions allow you to display a follow-up question only when a specific answer has been selected.

Example:

Main question: Would you like to move to a different position?

Available answers:

  • Yes

  • No

If the employee selects "Yes", a follow-up question appears: What type of position or geographic region are you interested in?

If the employee selects "No", the follow-up question is not displayed.

How to proceed

  1. Add a single-choice or multiple-choice question.

  2. Enter the available answer options.

  3. Add a conditional follow-up question.

  4. Define which answer will trigger its display.

  5. Save and preview the form to make sure everything works correctly.

Conditional questions allow you to create a personalized experience based on each participant's answers.

3. Customize question labels based on the participant's role

The same question can be worded differently for an employee and their reviewer.

Example:

  • Employee: "How would you assess your proficiency in the skills required for your position?"

  • Manager: "How would you assess your employee's proficiency in the skills required for their position?"

To customize question labels:

  1. Open the relevant question.

  2. Configure the title and, if necessary, the description displayed to the employee.

  3. Enter the title and description displayed to the manager.

  4. Save your changes.

This feature allows you to adapt the wording without creating two separate questions.

4. Define who should answer a question

For each question, you can select which participants should answer it:

  • Employee only: The question is intended for self-assessment.

  • Manager only: The question is reserved for the reviewer.

  • Employee and manager: Both participants can answer the question.

This configuration allows you to create forms tailored to each participant's responsibilities.

5. Make a question required or optional

When configuring a question, specify whether an answer is required.

A required question must be answered before the participant can proceed with the relevant step. An optional question can be left unanswered.

If a required field is left blank, an error message indicates that an answer is needed.

6. Review the form before using it

Before launching your review campaign:

  1. Make sure each section contains at least one question.

  2. Check which questions are required.

  3. Test the display conditions for follow-up questions.

  4. Verify the question labels displayed to employees and managers.

  5. Use the preview feature to check the participant experience.

Important: Once the campaign has started, the associated form becomes read-only. We therefore recommend finalizing its configuration before launching the campaign.
